Carolina Women’s Physicians closed our practice on 4/1/2025. It was our privilege to care for so many women and their families throughout our years in practice.

Prior to closing we notified patients by mail. This letter included a release of medical records. While we cannot release paper copies of records or fax records to other medical offices, we can provide patients with a copy of their records. These records can be provided in pdf form via email.

The carolina women’s physicians website will close on December 31, 2026. After that date, you can request your records by emailing jferguson@carolinawomens.com

The subject line should include patient’s name and request for records. Each request must be accompanied by a signed record’s release. These can be attached to the email request. Please include the patient’s full name, date of birth, and address in the email. This allows identification and satisfies HIPAA requirements for identification confirmation. Please allow 14 business days for record processing.
